The properties of Almagel are due to its constituent components:

  • Benzocaine, which is part of Almagel A, has a local analgesic effect.

The drug does not cause a sharp change in the acidity of the stomach in the direction of an alkaline reaction, and does not affect the pH of the blood, does not provoke the formation of stones in the biliary tract and does not disturb the electrolyte-water balance.

Almagel reacts with hydrochloric acid of the stomach, leading to a decrease in the digestive capacity of gastric juice, without causing its secondary hypersecretion. Mode of application

Antacids are intended for the treatment of acid-dependent diseases of the gastrointestinal tract by neutralizing the hydrochloric acid contained in the gastric juice. The active component of Almagel, aluminum hydroxide, has become the basis for the production of a number of antacids. Aluminum hydroxide, coming into contact with hydrochloric acid, neutralizes it by forming aluminum chloride. Almagel also contains magnesium hydroxide, which, in addition to neutralizing hydrochloric acid, eliminates the ability of aluminum chloride to provoke constipation. Active application Almagel in medical practice is due to the antacid, enveloping, gastroprotective and adsorbing properties of the drug. Thanks to D-sorbitol, which is part of Almagel, the drug is able to act as a laxative and enhance bile secretion.

The action of Almagel is observed for a long period of time. The drug does not secondarily increase the release of hydrochloric acid and does not cause flatulence, since the regulation of the acidity of hydrochloric acid occurs evenly without the formation of carbon dioxide bubbles. By stimulating the synthesis of prostaglandins, the remedy protects the gastric mucosa. Aluminum salts are absorbed into the intestine in a small amount, and magnesium ions - by 10%, but their concentration in the blood remains practically unchanged. The drug also contains benzocaine, which has almost no effect on the body, because it is absorbed in a minimal amount. The effect of Almagel for pain in the stomach occurs after 5 minutes, and its duration depends on the rate of gastric emptying. Yes, when using medicinal product on an empty stomach, the action lasts up to 60 minutes, and after application an hour after the last meal - up to 3 hours.

The drug is taken orally. Before each dose, the suspension must be thoroughly homogenized by shaking the vial.

Treatment

The drug is taken 45-60 minutes after meals and in the evening before bedtime.

Adults and children over 15 years of age are prescribed 5-10 ml (1-2 measuring spoons) 3-4 times a day. If necessary, a single dose can be increased to 15 ml (3 measuring spoons). Children aged 10 to 15 years are prescribed at a dose equal to half the adult dose.

Side effects

Green Almagel and yellow Almagel A in isolated cases, they lead to the development of nausea, vomiting, stomach cramps, changes in taste, epigastric pain and constipation. These side effects are quickly eliminated by simply reducing the dose of the drug. When used in high doses, the suspension may cause drowsiness as a side effect.

Long-term use may reduce the concentration of phosphorus in the blood, leading to bone destruction (osteomalacia). In patients with kidney failure Almagel can provoke swelling of the extremities, dementia (dementia) and an increase in the concentration of magnesium in the blood.

Almagel Neo when taken in medium doses, it can cause allergies, nausea, vomiting, constipation or diarrhea. Use in high doses can lead to the development of the following side effects:

Overdose

The so-called "acid rebound" - the development of secondary gastric hypersecretion - the drug does not cause. It has a local anesthetic, enveloping and adsorbing effect, neutralizes the influence of aggressive factors on the mucous membrane of the gastrointestinal tract. The drug begins to act after 3-5 minutes from the moment of administration and provides a persistent and pronounced therapeutic effect for 70 minutes. During all this time, Almagel A provides a stable neutralization of the constantly produced gastric juice and reduces the concentration of hydrochloric acid in it to the required limits. Aluminum hydroxide (algedrate) inhibits the release of pepsin, neutralizes hydrochloric acid with the formation of aluminum chloride, which, in turn, entering the intestine and swallowing the alkaline "spirit of freedom" there, turns into an alkaline salt. Magnesium hydroxide also neutralizes hydrochloric acid, forming magnesium chloride. The antagonistic attitude of magnesium hydroxide in relation to the tendency of aluminum hydroxide to cause constipation ensures the smooth functioning of the physiological "conveyor" of the digestive tract. Almagel neo is practically non-toxic and does not have teratogenic and mutagenic effects. In isolated cases in newborns whose mothers long time took the drug, there was an increase in tendon reflexes. In addition, in newborns there is a risk of a pathological increase in the amount of magnesium in the blood plasma, especially in a state of dehydration, and therefore long-term use of the drug for pregnant women and newborns is not recommended.

Almagel A is practically not absorbed in the gastrointestinal tract. The drug does not violate the water-salt balance and does not create any prerequisites for the appearance of alkalosis and other metabolic disorders. Almagel A does not irritate the urogenital tract and does not contribute to the formation of stones even with a long course of medication. Available as a suspension for oral administration. A measuring spoon is included in the package with the drug to ensure dosing accuracy. A single dose for adults is 1-3 scoops (their number is determined by the severity of the clinical case). Pharmacology

The instruction was approved by the Pharmacological Committee of the Ministry of Health of Russia on 03/02/2001.

Almagel A neutralizes free hydrochloric acid in the stomach, which leads to a decrease in the digestive activity of gastric juice. Does not cause secondary hypersecretion of gastric juice. It has a local anesthetic, adsorbing and enveloping effect, reduces the effect of damaging factors on the mucous membrane.

The therapeutic effect after taking the drug occurs after 3-5 minutes and lasts an average of 70 minutes.

Almagel A provides long-term local neutralization of continuously separated gastric juice and reduces the content of hydrochloric acid in it to the optimal limits for treatment. Aluminum hydroxide inhibits the secretion of pepsin, neutralizes hydrochloric acid, forming aluminum chloride, which in the alkaline environment of the intestine turns into alkaline aluminum salts. Magnesium hydroxide also neutralizes hydrochloric acid, turning into magnesium chloride. Thus, the effect of aluminum hydroxide, which causes constipation, is counteracted. Magnesium hydroxide and magnesium chloride are resorbed to a small extent and practically do not affect the concentration of magnesium ions in the blood.

Benzocaine has an effective and long-lasting local analgesic effect in severe pain syndrome.

Sorbitol, which is part of the preparation, promotes increased secretion of bile and exhibits a slight laxative effect, complementing the action of magnesium hydroxide.

Almagel A does not sharply increase the pH of the contents of the stomach, buffering its value from 4.0–4.5 to 3.5–3.8 (physiological value) between doses. The drug forms a protective layer, ensuring uniform distribution on the gastric mucosa active substances and provides long-term local action without the subsequent formation of carbon dioxide in the stomach, which, in turn, is the cause of flatulence, a feeling of heaviness in the epigastric region and a secondary increase in the secretion of hydrochloric acid.

According to the classification of Hodge and Sterner, the drug for oral administration refers to slightly toxic agents and does not have embryotoxic, teratogenic and mutagenic effects. In some cases, increased tendon reflexes were noted in newborns whose mothers took the drug for a long time. In addition, in newborns there is a risk of developing hypermagnesemia, especially in a state of dehydration, therefore, prolonged use of the drug by pregnant women and newborns is not recommended.

Pharmacokinetics

Almagel A is a non-absorbable drug. Subject to the correct dosing regimen and duration of treatment, it is practically not resorbed in the gastrointestinal tract and has a long-term uniform effect without disturbing the electrolyte balance and without creating a risk of alkalosis or other metabolic disorders. It does not irritate the urinary system and, with prolonged use, does not cause alkalosis and the formation of stones in the urinary tract.

Suspension for oral administration white or almost white color, with a characteristic smell of lemon; during storage, a layer of clear liquid may form on the surface, with vigorous shaking of the vial, the homogeneity of the suspension is restored.

Dosage

Interaction

With the simultaneous use of Almagel A, it can reduce the therapeutic effect of tetracycline antibiotics, histamine H 2 receptor blockers, digitalis glycosides, iron salts, ciprofloxacin, phenothiazines, isoniazid, beta-blockers, indomethacin and ketokenazole, etc.

Side effects

In some cases, when taking the drug, there may be a change in taste sensations, nausea, vomiting, stomach cramps, pain in the epigastric region and constipation, which disappear after a dose reduction. At high doses, it may cause drowsiness.
Long-term treatment with the use of high doses of the drug and food poor in phosphorus can lead in predisposed patients to the development of phosphorus deficiency in the body, increased resorption and excretion of calcium in the urine and the occurrence of osteomalacia. Therefore, with prolonged use of the drug, it is necessary to ensure sufficient intake of phosphorus with food.
In patients with chronic renal failure, in addition to osteomalacia, edema of the extremities, dementia and hypermagnesemia can be observed.
